Job Title: Home Health RN / LPN
Overview:
Join our team as a Home Health Skilled Nurse: RN or LPN.
Duties:
- Traveling within Coos County to patients homes to provide Skilled Nursing services. Traveling areas include: Bandon, Lakeside/Hauser, Coquille, North Bend and Coos Bay.
- Skilled Nursing assessment and treatment plans to include but not limited to: PICC, PORT, SP/Urethral Catheter, Wound Care, Disease education, Caregiving education and training.
- Working within an inter disciplinary team to include: Skilled Nursing, Physical Therapy, Occupational Therapy, MSW and HH Aide.
Experience preferred:
- Home Health
- Hospice
- Case management
- Oasis
- EMR: Wellsky
Requirements:
- Licensed Registered Nurse or Licensed Practical Nurse in Oregon.
- Strong communication and interpersonal skills
- Compassionate attitude and empathy towards clients
- Ability to work independently and as part of a team
- Reliable transportation to travel to client homes
