What are the responsibilities and job description for the Community Health Worker - Eugene Oregon Clinical Location position at Oregon Integrated Health?

Oregon Integrated Health is a Primary Care Medical Home.  We have clinic locations in Portland, Eugene, Salem and Florence Oregon. Oregon Integrated Health has providers that include Medical Doctors, Naturopathic Physicians, Family Nurse Practitioners, Psychiatric Mental Health Nurse Practitioners, Counselors, Chiropractors, Acupuncture, Physical Therapy, Massage Therapy and Eugene has an Expanded Practice Dental Hygienist for our oral health program. 


We will be interviewing CERTIFIED AND UNCERTIFIED Community Health Worker.  Uncertified applicants will need to complete the Oregon Traditional Health Worker Certification. and be registered with the Oregon Health Authority. 


Position will build working relationships, solve problems and support patients while they learn to self-navigate the health care system. The job is challenging and rewarding and requires that you clearly understand your role and responsibilities.  Position collaborates with the Clinical Providers, Medical Assistants and Scheduling Team for patient care.  Job Position will be supervised, and job tasks are to be performed related as assigned by Oregon Integrated Health through guidelines, policies and workflows.

 

 

EXAMPLES OF DUTIES: Duties may include but are not limited to the following as well with guidelines enlisted in Clinical Policy Guidelines and OIH Workflows.

  • Gather, analyze and report patient health data related to quality health metrics for our patient demographic.
  • Scheduled in person or telemedicine visits with patients to improve access to healthcare and social services
  • Design and implement a plan with care team to improve patient health related to health data
  • Identify high risk individuals at the clinic and assess for chronic diseases,
  • Management of complex care needs of patients – providing access to services, resources, a provide knowledge with the care team to help patients get the care they need.
  • Assesses the patients’ social determinants of health, utilizing screening tools and questionnaires to assess the patient’s health status, safety and healthcare needs.
  • Acts as the patient’s advocate and health navigator, assisting them in understanding the most appropriate use of health care resources.
  • Will also work with the Primary Care Teams to outreach to patients with complex health and those who are high utilizers of hospital and Emergency Department services to remove barriers to care.
  • Documents all notes relating to patient care in EHR

 

MINIMUM QUALIFICATIONS

  • High School diploma/GED required; Associates degree preferred.
  • Six months medical documentation related experience or training required. 
  • Proficiency in Microsoft Office Suite required.
  • Ability to work in a fast-paced environment and juggle multiple priorities.
  • Able to think quickly, assess a situation and make a sound decision.
  • Able to use technology to optimize efficiency and effectiveness.
  • Solid written and verbal communication, listening, organization and priority setting skills.
  • A track record of being consistent with attendance and First aid.

 

Ability to:

  • Understand, analyze and transmit information effectively; receive and act upon verbal and written information; learn assigned tasks quickly; anticipate patient and provider needs.
  • Prioritize work and practice time management to ensure timelines are met and the needs of multiple clients are addressed.
  • Maintain accurate records for medical charts and reports; develop and maintain professional, effective, working relationships with supervisors, peers and clients.
  • Communicate clearly and concisely, both orally and in writing.

 

PAY

$20.00 - $21.50 based on experience and THW certification
