Position Summary:

Are you passionate about making a difference in the lives of others while working in a supportive, mission-driven environment? Join our team as a Residential Care Coordinator, 40 hours Monday-Friday, and play a vital role in empowering individuals on their journey toward recovery and independence.


Under the guidance of the Clinical Nurse and Residential Manager, you’ll collaborate with an interdisciplinary rehabilitation team to develop and oversee individualized treatment plans. As a key advocate for our clients, you’ll ensure access to the services they need—both within and beyond our organization—while maintaining a balance between flexibility, productivity, and exceptional quality care.


The Residential Care Coordinator serves as a bridge between clients, families, and other stakeholders, fostering open, effective, and timely communication that reflects Origami's mission, vision, and values. Responsibilities:

  • Collaborate with team members to develop and implement individualized treatment, transition, and discharge plans that enhance outcomes and track quality metrics.
  • Participate in the transition of responsibilities from admissions or other care coordinators to ensure continuity of care.
  • Serve as an organizational ambassador by fostering professional relationships and delivering consistent customer service to stakeholders.
  • Resolve client and support system concerns and maintain up-to-date knowledge of clients on assigned caseloads.
  • Coordinate and maintain timely and thorough client documentation, admission paperwork, and billing in compliance with regulations and funding requirements.
  • Oversee client fund management, including tracking, reconciliation, and communication with external stakeholders.
  • Facilitate communication regarding respite, leaves of absence, and visitor needs while processing client mail efficiently.
  • Assist in developing and implementing program activities and community guidelines to support consistent and effective treatment goals.
  • Coordinate external rehabilitation services, including scheduling, logistics, follow-ups, and community resource identification, to enhance therapeutic outcomes.
  • Provide on-call supervisory support as part of a rotation to ensure the seamless operation of 24/7 programming.


Requirements:

  • An associate degree in a related area; or equivalent employment experience may be considered in lieu of a degree.

Preferences:

  • Knowledge of personal computers and proficiency in Microsoft Office Applications.
  • A minimum of two years of related experience in rehabilitation and/or case management.



Physical Requirements:

While performing the duties of this job, the employee is regularly required to talk and hear. The employee is occasionally required to stand, walk, sit, use hands to finger, handle or feel, reach with hands and arms, climb or balance, stoop kneel or crawl, and taste or smell. Accommodated alternatives are acceptable if they accomplish the necessary essential tasks. The employee may occasionally lift and/or move up to 40 lbs. Specific vision abilities required for this position include close vision, distance, peripheral vision, depth perception, and ability to adjust focus.
