What are the responsibilities and job description for the Behavioral Health Navigator position at OSF HealthCare?
Expected pay for this position is $24.61 - $28.96/hour. Actual pay will be determined by experience, skills and internal equity.
POSITION SUMMARY:
The Behavioral Health Navigator assists patients/families, agencies, and OSF healthcare staff by identifying resources for patients with behavioral healthcare needs. The Navigator is responsible for appropriate use of resources to ensure our patients receive care that is continuous, comprehensive, and coordinated across the care continuum. Works closely with community resources and serves as a community resource knowledge expert. Assists in meeting regulatory requirements and current contractual obligations.
Qualifications
REQUIRED QUALIFICATIONS:
Education:
Bachelor's in Psychology, Social Work, Sociology, or other related human services field
Experience:
1 year of behavioral health experience
Other skills/knowledge:
Excellent interpersonal and communication skills.
Solid computer skills, including proficiency with Microsoft software.
Strong analytical and problem solving skills, with the ability to be detail oriented.
PREFERRED QUALIFICATIONS:
Other requirements/information:
Proficiency with use of Electronic Medical Record (EMR).
Able to recognize crisis situations to elevate to management/clinicians/Emergency Department, Emergency Response Service or other Community Emergency Resources.
