Expected pay for this position is $26.39 - $31.05/hour. Actual pay will be determined by experience, skills and internal equity. This is a Salaried position.
Overview
This job is located in Evergreen Park,IL
Monday - Friday 8:00- 4:30pm and some rotating Saturday mornings.
POSITION SUMMARY: The Complete Care Navigator works within a care team, acting as an advocate, and coordinates care for OSF patients. Creates a seamless patient experience by serving as a resource with expertise in the care journey, proactively engaging the patient, and anticipating patient needs. The Complete Care Navigator participates in care team huddles by providing information to align care team of patients' needs. Provides direction and shepherds the patient throughout their encounters to appropriate specialties, supportive services, community resources internal and external to OSF Healthcare.
Qualifications
REQUIRED QUALIFICATIONS: Education: High School Diploma/ GED Experience: 2 years of work experience in a healthcare setting and/or patient care setting Other Skills/ Knowledge: Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software. Strong analytical and problem-solving skills, with the ability to be detail oriented. Ability to be detail oriented and experience with problem solving and conflict management.
PREFERRED QUALIFICATIONS: Education: Associate's degree in a healthcare, business, or related field Experience: 3 years of experience in an ambulatory clinic Prior experience with Electronic Health Record Platforms (Epic preferred) Experience with case management, social work, patient advocacy, or customer service Licensure/ Certification: Medical Terminology Course Other Skills/ Knowledge: Knowledgeable with insurance especially Medicare
