What are the responsibilities and job description for the Hospice Liaison position at OSF HealthCare?
Starting pay for this position is $23.84/hour; base pay offered may increase based on experience, skills and internal equity.
POSITION SUMMARY: The Hospice Liaison in conjunction with the Leadership team acts as a liaison with hospice referrals and referral sources. The Liaison assists in identifying referral sources as well as coordinating hospice referrals. In addition the Liason will make on site visits to referral sources and to homes of identified hospice referrals.. May facilitate advanced care planning with the family. The Liaison will effectively prioritize and respond appropriately to various issues identified by recommending effective resolutions, conferring with providers and collaborating with the Regional Leadership to ensure that the response and care identified is managed to meet the patient's need. The Liaison provides personal and telephonic guidance, and education with the goal to improve the patient, provider, and referral source experience.
Qualifications
REQUIRED QUALIFICATIONS:
Education: Bachelors
Other Licenses/Certifications: BLS - American Heart Association must obtain within 90 days from date of hire.
Experience: 2 years of healthcare or social service experience
Other skills/knowledge: Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software. Strong analytical and problem solving skills, with the ability to be detail oriented.
PREFERRED QUALIFICATIONS:
Experience: Hospice/Home Care experience.
Education: Bachelor's in healthcare related field, counseling or business administration
