What are the responsibilities and job description for the Plan Coordinator position at Oswego Industries, Inc.?
SUMMARY: Responsibility for the creation of and oversight of implementation of habilitative and/or vocational plans to support identified goals of assigned program participants. Plan development includes engagement with support team and a focus on experiences which support individual choice, independence and community integration.
ESSENTIAL DUTIES and/or RESPONSIBILITIES include the following.
1. Attend participant annual and semi-annual individualized planning meetings.
2. Develops, implements, and uploads participant plans and objectives within regulatory timeframes.
3. Document individual safeguards and update as necessary.
4. Responsible for collection and distribution of plans and eligibility information.
5. Collects and maintains attendance data per agency policy and billing standards
6. Train Direct Support Professionals on updates, and/or changes to programs and safeguards.
7. Provides direct care back-up coverage in all Habilitation Programs as needed.
8. Write program notes for assigned individuals supported.
9. Review and quality control of daily billing documentation as assigned.
10. Incorporate choice and informed decision making into all phases of planning.
11. Assess strengths and barriers to individuals’ success and develop strategies to promote success.
12. Collaborate with Nursing and Clinical team as participants’ needs require.
13. Provide support to program staff in implementing goals and programs in site-based and community settings.
14. Work as part of a cohesive interdisciplinary team to support participants’ engagement in program and goal achievement.
15. Augment current job knowledge by participating in conferences, seminars, workshops, etc., conducted both within the agency and outside the agency.
16. Maintains confidentiality of all persons served and contributes to the improvement of agency operations.
17. Acts in accordance with policies, procedures, and the Agency Corporate Compliance Policy. Reports suspected or actual violations thereof to the appropriate party(s).
18. Other duties as assigned by the Program Manager or Associate Executive Director for Program.
QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE: Associates Degree in Human Services or related field of study with 1 - 3 years’ experience working in a disability services agency or similar program. Or High School diploma (or equivalent) and combination of education, training and 2-5 years’ experience working with people with disabilities. A valid, clean New York State driver license required
LANGUAGE SKILLS: Ability to read and interpret general business periodicals, professional journals, or government regulations. Ability to write reports, business correspondence, and procedural manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
REASONING ABILITY: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
MATHEMATICAL SKILLS: Ability to calculate basic figures and amounts such as portions, percentages, etc
