What are the responsibilities and job description for the Registered Nurse Case Manager position at Our Hospice of South Central Indiana?
Our RN Case Manger's work with a caseload of patients with the help of Nurse Practioners, Physicians, Aides, Chaplains, and Social Workers. Our patients and families are in the most important time of their life and need a skilled nurse to help navigate their pathway. In this position, everyday is a little different and you are on the go visiting patients in your community.
What you need for this position:
- Diploma, Associate, Bachelor Degree in Nursing
- Current Registered Nurse License in Indiana
- Drivers License
- Interest and Ability in providing patient care in homes and facilities
- Ability to travel in our service area (see map in photos)
What you want from this position:
- Work-life balance, Full-Time flexible scheduling Monday- Friday
- 8 am- 4:30 pm.
- Salary for this position fluctuates based on level of education and experience. Starting at $35/hour.
- Mileage- $0.52/mile
