About the Position
We seek a dedicated and experienced Director of Community Hospice and Home Health Care to lead and manage the daily operations of our Community Hospice and Home Health Care service lines. The DIrector establishes, implements and evaluates goals and objectives for hospice and home health care services that ensure compliance and promote the standards of quality care of the day-to-day operations.
This role ensures the delivery of high-quality facility services while maintaining compliance with all applicable regulations and standards. Responsibilities:
- Supervises and directs clinical supervisors in managing and coordinating the clinical teams to ensure quality and compliant care.
- Evaluate performance of supervisors as well as recruit, onboard and lead employees.
- Oversee patient clinical records, reports, data trends for reporting and for Quality Assurance and Performance Improvement (QAPI).
- Collaborates with the Performance Improvement Coordinator and Education Coordinator for education and training to ensure compliance, standards of practice and continuing education to meet the needs of the patients.
- Responsible for census development and assist with collaborative partnerships.
- Develop and maintain clinical policies related to community hospice and home health care.
- Review and maintain all service contracts with outside agencies.
- Willing to work a flexible schedule, including evenings and weekends as needed and be on call during non-working hours.
Qualifications:
- Must be Registered nurse in MN with current license to practice professional nursing in the state.
- Must have 3-5 years management experience in hospice or related health care organization.
- Bachelor's degree in nursing and preferred Masters degree in nursing or related field.
- Knowledge of business management, regulations and industry standards.
- Strong leadership, communication, and problem-solving skills.

- Must successfully pass a criminal background check.

Position Details:
This position is 80 hours per two-week pay period and reports to the Director of Operations. The salary range is $55.00 to $64.50 per hour.
