- Assists and advocates on behalf of the individual; upholds respectful and efficient communication with individuals and all relevant parties.
- Provides emotional support by developing an age-appropriate relationship with individuals.
- Assists individuals in experiencing all phases of the Pathways Program, both on-site and off-site including transportation, event participation, learning, re-learning programs and socialization opportunities.
- Aids individuals in developing and maintaining daily living skills, encouraging independence, personal hygiene, behavior (i.e., appropriate responses / actions with others including other individuals and the public), proper dietary and mealtime habits, and safety in and out of the community.
- Performs direct support and supervision, including but not limited to :
o Providing transportation for social activities and other appointments.
o Providing individuals health care as needed and following any relevant orders, BSPs, ISPs or IHPs
o Adhering to individuals' rights, mandated reporting, and unusual incident requirements.
o Providing supervision during community and recreational outings.
o Attending individual-related meetings and appointments as needed.
o Observing any changes in individual status / well-being and reporting immediately to appropriate individuals.
o Demonstrating an understanding of procedures for all emergencies, including medical, transportation, natural disaster, fire and unusual incidents.
Obtains and maintains certification as a Medication Passer.Attends appropriate in-services and continuing education.Maintains strict confidentiality of individual and personnel information.Collects and records data regarding individual well-being, progress, health, unusual incidents, personal goals and other information.Understands and complies with job requirements as outlined in Agency policies / procedures, e.g., mandated reporting, personnel policies, individual rights, etc.Gives input and information regarding individuals' individual plans, health concerns and behavioral plans.Completes documentation daily and communicates with staff, individuals, family, medical professionals, and other personnel in reference to individuals.Demonstrates a strong commitment to the mission and values of the agency.Adheres to agency's attendance standards.Performs other related duties as assigned.Adult Program Specialist Minimum Qualifications and Requirements :
Eighteen (18) years of age.Must have a valid Ohio Driver's License with a record suitable for insurability purposesDriver's license must have less than the maximum number of points per agency policyMust have reliable transportation and two years minimum driving experienceMust provide proof of a High School Diploma or equivalentCriminal background that is compliant with OAC 5123 : 2-2-02 Employment of persons providing services to individuals with DDMust be compliance with agency drug free workplace programMust have the ability to communicate and interact with individuals, families, and staff maintaining the highest level of respect and serviceMust have the ability to deal with pressure, problems, and difficult situations, while maintaining perspective and emotional stabilityAbility to complete all required training.Cooperative, patient, and caring nature.Communicate and interact with individuals, families, and staff, maintaining the highest level of respect and service.Ability to participate in all in-service trainings and a minimum of 8 hours annual continuing education.Ability to participate in orientation and on the job training.Dependable, demonstrated adequate writing skills, good moral character and able to work in stressful situations in a calm, therapeutic manner.Experience working with individuals with Developmental Disabilities is preferred.Environmental Factors / Physical Requirements :
May be exposed to infectious illnesses.May be exposed to aggressive individuals.Requires physical exertion including but not limited to walking, bending, reaching, kneeling, squatting, pushing, pulling, and lifting 50 lbs. or more. These may be in excess of light duty work.Requires corrected hearing and vision to normal range.
