You will adeptly assess and complete the Person-Centered Planning Tool (PCPT) while collaborating with individuals, their families, and team members to set goals, ascertain eligibility, and identify necessary services using the NJCAT to pinpoint support needs. Your expertise in coordinating and facilitating off-site team meetings at residential and day treatment programs will help craft individualized support plans (NJISP) tailored to personal, medical, nutritional, health, safety, and employment issues. You'll connect families with essential resources while maintaining up-to-date electronic records and ensuring compliance with Medicaid requirements. Your commitment to quality case management will include consistent communication with individuals and their support teams, ensuring all objectives are met efficiently while actively participating in training and performance reviews to uphold the highest standards of care.
