What are the responsibilities and job description for the Community Health Worker position at OZARKS COMMUNITY HEALTH CENTER?
We are seeking a current Missouri Community Health Worker to join our family of professionals, with a heart to make a difference in the underprivileged communities surrounding our locations.
We want to pair with the perfect candidate to enhance our services to better serve our patients with the best quality, compassionate and professional health care to all who enter our doors.
Requirements:
· High School Diploma or equivalent.
· Computer skills or willing to participate in PC training.
· Certified in BLS CPR.
The Community Health Worker will support the Mission of Ozarks Community Health Center and work within the Care Team to facilitate patient goals and connect patients with needed resources in the surrounding community.
Communicates with a specified number of patients on a scheduled basis both in person (clinic or in home) and via phone.
Facilitate needed appointments.
Follow up on appointments, etc.
Participate in Health Home staff meetings and trainings.
Assist with and advance patient self-management,
Communicates with patients, families and providers to keep entire team focused on meeting patient center goals.
And behaves in a manner consistent with the mission and objectives of OCHC and performs other duties as requested.

Pay: $25,000.00 - $35,000.00 per year

All employees and patients, including family members and visitors must wear masks and practice social distancing as applicable. We also participate in vaccine clinics both onsite and offsite.
