What are the responsibilities and job description for the Home Care Coordinator position at PACE@Home?
PACE@Home is seeking a Home Care Coordinator to fulfill the organizations need to develop and implement homecare services for program participants, including the coordination of all durable medical equipment, medical supplies, home health and nursing home care.
Employee Type: Full-Time; Monday-Friday 8:30 am - 5:00 pm
Minimum Experience: At least on year experience working with the frail and elderly population. Three to five years home care experience preferred.
Required Degree: Graduate of an accredited school of nursing – Bachelor’s degree preferred. Current, unrestricted North Carolina nursing license required. Must maintain CPR and First Aid Certification.
Other Requirements: Must have Medical form filled out and signed by physician, PA or NP prior to working.
Travel: 75%
