To provide agency food services and counseling and teaching services to participants and caregivers to promote optimal nutrition within the constraints of the patients’ illness and plan of care.

Essential Duties and Responsibilities:

  • Oversee, plan and direct the food and nutritional services programs for PACE @ Home.
  • Provide appealing and healthy meals that encourage meal consumption to participants
  • Monitor food preparation methods, portion sizes, and garnishing and presentation of food in order to ensure that food is prepared and presented in an acceptable manner.
  • Monitor budgets and payroll records, and review financial transactions in order to ensure that expenditures are authorized and budgeted.
  • Arrange for equipment maintenance and repairs, and coordinate a variety of services such as waste removal and pest control with Center Manager.
  • Monitor compliance with environmental health regulations to ensure safe delivery of food.
  • Monitor compliance with fire/safety regulations regarding food preparation.
  • Monitor compliance with all county, state and federal regulations that govern nutritional service.
  • Actively participates in the quality evaluation, assessment and improvement process.
  • Serves as a member of the QAPI committee and chair of the weight committee.
  • Coordinate assignments of cooking personnel in order to ensure economical use of food and timely preparation.
  • Performs an initial assessment on all dietary referrals to determine patient and caregiver educational needs.
  • Develops nutritional plans within the scope of PACE @ Home.
  • Provides nutritional counseling services as outlined in the Plan of Care including instruction to the patient and caregiver(s) and recommendations for adaptations.
  • Re-evaluates/revises the plan of care as needed with input from the Interdisciplinary Team.
  • Provides in-service education, educational tools, and consultation regarding nutritional issues to staff as requested.
  • Assists with the development and review of nutritional care policies and procedures consistent with Academy of Nutrition and Dietetics standards.
  • Ensures that documentation is consistent within established standards.
  • Demonstrates familiarity with policies of PACE @ Home; understands and adheres to rules, regulations and standards of regulatory bodies, QAPI program and applicable accrediting agencies.
  • Complies in a timely manner with all mandatory inservice requirements of PACE @ Home.
  • Accepts other assignments as appropriate.

Supervisory Responsibilities

Responsible for supervising dietary staff. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

Education:

Current licensure as a Registered Dietitian; Current registration with the Academy of Nutrition and Dietetics.

Experience:

Must have at least one year of experience as a dietician working with participants who are 55 or older.

Skills/Abilities:

  • Excellent verbal and written communication skills
  • Ability to manage time efficiently and prioritize duties.
  • Ability to work with people from different cultures, ethnic backgrounds and religious affiliations.
  • Consistently demonstrates responsibility, accountability and dependability.
  • Proficient computer skills with MS Office products.
  • Excellent customer service and communication skills.
  • Maintains current, unrestricted driver’s license.
  • Maintains minimum automobile liability insurance for duration of employment.

Physical Requirements:

  • Must be able to perform basic life operation skills of walking, sitting, standing, bending, reaching, twisting, talking and hearing.
  • Requires crouching/squatting, kneeling. Hand/finger dexterity and writing.
  • Must be able to perform sedentary work; and to occasionally move objects up to 50 pounds.
  • Must possess the visual acuity to operate a computer terminal and to read and write extensively.

Job Type: Full-time

Pay: $56,000.00 - $84,000.00 per year

Schedule:

  • 8 hour shift
