Job Summary:

The PACE Center Manager is responsible for overseeing the daily operations of the PACE center, ensuring quality care and services for participants. This role involves managing staff, coordinating care services, and ensuring compliance with regulatory standards.


Essential Responsibilities:

  • Oversee the management of the PACE center’s daily operations to ensure efficiency, quality, and compliance with PACE regulatory requirements.
  • Manage and supervise center staff, including hiring, training, coaching, performance reviews, and termination of employees.
  • Foster effective teamwork and performance, manage change, and encourage innovation. Build collaborative relationships, encourage involvement and initiative, and develop goal orientation in others.
  • Oversee the provision of adult day health services at the center, including food services, social activities, and transportation.
  • Ensure compliance with all CMS/State regulations to maintain a safe facility for participants and staff.
  • Lead the initiation and implementation of PACE process/performance improvement activities. Collaborate with key stakeholders to improve processes, utilizing visual boards and frequent huddles to monitor key performance indicators and identify improvement opportunities.
  • Ensure consistent workflows and documentation to track program success and outcomes.
  • Collaborate with healthcare professionals as a member of the interdisciplinary team to develop and implement individualized care plans for participants.
  • Develop policies and procedures for review by Senior Leadership as needed.
  • Assist in the development of annual center budgets, oversee spending throughout the year, and take corrective action as needed.
  • Organize and coordinate activities and programs to enhance participant engagement and satisfaction.
  • Actively participate as a key team member in Manager/Supervisor meetings and other internal committees, sharing pertinent information.
  • Facilitate communication between participants, families, paid caregivers, residential staff, and the interdisciplinary team (IDT).
  • Work closely with PACE senior leadership to develop and implement new programs and processes to support the ongoing success of the program’s goals and initiatives.
  • Oversee and monitor processes to ensure the protection of personal health information.
  • Monitor the quality of services provided and implement improvements as needed.
  • Serve on various internal and external committees as required or designated.
  • Conduct regular meetings with staff and stakeholders to discuss center operations and participant care.
  • Foster a safe and supportive environment conducive to the professional development of healthcare professionals.


Supporting Responsibilities:

  • Meet department and company performance and attendance expectations.
  • Follow privacy policy and HIPAA laws and regulations concerning confidentiality and security of protected health information.
  • Perform other duties as assigned.


SUCCESS PROFILE


Work Experience: A minimum of five (5) years of leadership experience in a healthcare setting, along with one (1) year of experience working with a frail or elderly population is required. Experience in PACE or other Long Term Services and Supports settings is preferred.


Education, Certificates, Licenses: Licensed Clinical Social Worker, Registered Nurse, Occupational Therapist, Recreational Therapist, or Physical Therapist with unrestricted Oregon license required.


Knowledge: Employee(s) must possess strong leadership, communication, and interpersonal skills. Proficiency with computer information systems and software is essential. The ability to work effectively with individuals who have diverse health issues, concerns, and personalities is crucial. Strong analytical and problem-solving skills are necessary. They should be proficient in the use and implementation of tools and concepts such as Training, Coaching, Strategy Deployment, Daily Operations, Visual Management, Operational Improvement, and Team Building/Development across all teams within their scope and accountability.


Each member of the PACE organization's staff that has direct contact with participants must meet the following conditions:

  • Be legally authorized (for example, currently licensed, registered or certified if applicable) to practice in the State in which he or she performs the function or action.
  • Only act within the scope of his or her authority to practice.
  • Have 1 year of experience working with a frail or elderly population
  • Meet a standardized set of competencies for the specific position description established by the PACE organization before working independently.
  • Be medically cleared for communicable diseases before engaging in direct participant contact.

Environment: Work inside in general office and clinical settings with ergonomically configured equipment. Travel is required approximately 10% of the time.
