Position Overview:PanCare Health is seeking a compassionate and dedicated Diabetes Educator (RN) to join our team. This role is ideal for a Registered Nurse with a strong passion for diabetes education and patient empowerment. The Diabetes Educator will play an integral part in helping patients manage their diabetes, improve their health outcomes, and adopt healthier lifestyles.
The role involves collaborating with healthcare teams to deliver individualized care, provide education on diabetes self-management, and support patients in understanding and navigating their condition. A successful candidate will be skilled in patient communication, education, and collaborative care, while demonstrating a commitment to patient-centered health practices.
Key Responsibilities:
Diabetes Education & Training
- Assess program participants using the AADE7 framework, collaborating with patients to set personalized learning goals and objectives.
- Develop individualized educational plans, incorporating interactive, skill-based teaching methods.
- Provide ongoing support and guidance to program participants, focusing on collaborative, hands-on learning.
- Evaluate the effectiveness of educational interventions by assessing the attainment of learning objectives and overall participant progress.
- Conduct follow-up assessments and document the results, using outcome measures from the AADE7 Continuum of Outcomes Framework.
Patient-Centered Care
- Develop and implement individualized follow-up plans for patients after the completion of diabetes education services.
- Collaborate with healthcare team members (physicians, dietitians, etc.) to ensure comprehensive care for each patient.
- Identify when additional services are required beyond the educator's scope and coordinate referrals accordingly.
- Maintain regular communication with primary care providers about participant progress and needs.
Quality Improvement & Professional Development
- Contribute to continuous quality improvement initiatives, aiming to assess and enhance the effectiveness of the DSME/T program.
- Participate in peer review processes to evaluate the performance of fellow diabetes educators.
- Regularly assess personal performance and set goals for continued professional growth.
- Maintain at least 15 hours of continuing education annually in diabetes-related topics, self-management education strategies, and behavior change techniques.
Documentation & Administrative Duties
- Maintain accurate and up-to-date records for each program participant, documenting assessments, educational interventions, and follow-up results.
- Contribute to the development of educational materials for program participants and other healthcare providers.
Knowledge, Skills, and Abilities:
- Expert Knowledge in Diabetes Management: Deep understanding of current diabetes treatment protocols, self-management strategies, and behavioral change techniques.
- Education & Facilitation Skills: Ability to lead individual and group education sessions, creating a supportive and collaborative environment for learning.
- Effective Communication: Strong ability to connect with individuals, fostering a therapeutic relationship to ensure active participation and engagement in their care.
- Organizational & Documentation Skills: Proficient in documenting and managing patient information in compliance with healthcare standards.
- Computer Skills: Basic proficiency with email, internet usage, and electronic health records.
Qualifications:
- Registered Nurse (RN) with active licensure.
- Experience in diabetes education or a related healthcare field is required.
- Knowledge of the AADE7 framework and DSME/T programs is a plus.
Physical Demands:
- Mobility Requirements: Frequent standing, walking, and occasional sitting. Ability to reach, bend, kneel, and squat as required.
- Lifting: Occasionally lift and/or move up to 30 pounds.
- Emotional Resilience: Must be able to maintain a professional and calm demeanor in challenging situations.
