This vital role involves assessing patient needs, providing counseling, and coordinating community resources to support our patients’ health and independence.
Responsibilities
- Works as a team member with all departments.
- Demonstrates critical thinking and decision making abilities in assessment, treatment planning, medical social services intervention, and evaluation.
- Coordinate community resources and support services.
- Documents on all required forms and ensures confidentiality of all patient and family information.
- Provides coordination, referral, consultation, and education.
- Works as a Liaison to hospitals and skilled nursing facilities.
Qualifications
- Valid WV State licensure as a Medical Social Worker.
- Bachelor or Masters Degree in Social Work.
- Preferred one year experience in home health or hospital settings.
- Knowledge of HIPAA regulations and medical documentation.
- Excellent communication and interpersonal skills.
- Ability to handle sensitive situations with compassion.
- Experience working with diverse populations including elderly, children, or disabled individuals.
- Knowledge of mental health issues such as depression, dementia, PTSD.
