PHYSICAL THERAPIST - PARKVIEW HEALTH
Eligible for Sign on Bonus!
Locations available in Fort Wayne, Huntington, Warsaw, Kendalville, Columbia City, and Auburn plus a position in Bryan, OH as well.
Schedule :
Hours : 40 a week
Shift : Days
Weekends : None
Qualifications :
Must be a graduate of an accredited school of physical therapy.
Degree Requirements :
Bachelor’s degree required if graduated before 1 / 1 / 2016.
Doctorate of Physical Therapy required if graduated after 1 / 1 / 2016.
International candidates must have a Bachelor’s degree in Physical Therapy (Doctorate preferred).
Licensing :
Must hold or be eligible for a Physical Therapist license in the state where services will be provided.
State licensure must be obtained within the first 90 days of employment.
Certifications :
Must be certified in CPR or obtain certification within the first 90 days of employment.
Must maintain all certifications and licenses as required.
Preferred :
Membership in the American Physical Therapy Association (APTA) is preferred.
Driving Requirements :
If assigned to drive a company vehicle, a valid driver’s license is required.
If using a personal vehicle, proof of insurance and a valid driver’s license are required.
Physical Requirements :
Must be able to lift 50 pounds, assist with lifting up to 100 pounds, and push up to 300 pounds.
Skills :
Strong verbal and written communication skills are essential.
Some positions require the ability to go to different work locations as needed.
