Position Description
Parsons Counseling & Play Therapy Center is seeking Targeted Case Managers (TCMs) to provide resources and case management services for individuals with serious mental illness (SMI), severe emotional disability (SED), substance use disorder (SUD), or co-occurring SMI/SED/SUD and chronic/complex physical health conditions. TCMs assist clients in gaining access to needed medical, social, educational, or other services and supports through assessment, service planning, linking to services, monitoring, and documentation. TCMs also represent the agency at community meetings and events to learn about local resources available to connect clients to services. Marketing and outreach are key parts of this role.
Services may be home, school, community, or office-based. Some travel is required. We serve Madison County and the surrounding area, with locations in Berea and Richmond, KY.

Position Responsibilities
- Provide quality support services that are in accordance with agency, state and professional standards
- Complete clinical documentation in a timely manner
- Participate in staff meetings and in-service training
- Coordinate with other staff and the services director
- Complete additional training as required by the state
- Conduct comprehensive assessments of clients' medical, social, functional and behavioral health status
- Identify client strengths, needs and desired services/supports
- Develop individualized service plans and arrange access to chosen services
- Facilitate engagement in services by explaining importance relative to client needs
- Monitor service access, quality and delivery through ongoing contact
- Maintain comprehensive documentation including assessments, service plans, progress notes, etc.
- Complete required training and continuing education for TCM certification
- Participate in supervision per certification requirements
- Attend staff meetings, trainings and community meetings/events as scheduled
- Assist with marketing/outreach efforts for the agency
Minimum Qualifications
- Bachelor's degree in behavioral science field (psychology, sociology, social work, counseling, nursing, etc.)
- 1-year full-time experience working directly with adults or children/youth in a human services setting, post Bachelor's degree
- Successful criminal background check
- Ability to obtain TCM certification within 6 months through approved training
- Commitment to obtaining continuing education to maintain TCM certification
