Position Overview
We’re seeking a passionate Psychotherapist to join our team at Partners In Hope LLC - Psychiatry. In this role, you’ll deliver essential psychotherapeutic services to residents in skilled nursing and assisted living communities. Through individualized care plans and ongoing therapy, you’ll help patients navigate emotional, psychological, and social challenges, improving their overall quality of life.
Your Responsibilities
- Conduct comprehensive initial assessments to evaluate mental health and psychosocial needs.
- Create individualized treatment plans tailored to patient goals.
- Provide ongoing psychotherapy to address emotional and psychological challenges.
- Collaborate with interdisciplinary teams and facility staff to ensure seamless continuity of care.
- Maintain accurate and compliant documentation in accordance with state and federal guidelines.
What You Bring to the Table
- A master’s degree in social work or counseling from an accredited institution.
- An active, unrestricted license in the state of Illinois.
- Expertise in psychotherapeutic treatment modalities and mental health diagnoses.
- Exceptional communication skills and a collaborative spirit, with the ability to work effectively with team members, facility staff, patients, and families.
