The Healthcare Assistant plays a vital role in supporting the medical needs of adults with intellectual and developmental disabilities, ensuring they receive the highest quality of care. Reporting to the Director of Nursing, this position is responsible for coordinating, attending, and documenting medical appointments, managing medications, and ensuring the continuity of care for individuals supported by Pathfinder.


The Healthcare Assistant works directly with individuals to provide transportation, guidance, and reassurance during medical visits. They also serve as the liaison between healthcare providers, families, and interdisciplinary team members, ensuring clear communication and documentation of all medical instructions, results, and follow-up actions.


This role requires strong organizational skills to manage routine and emergency appointments, maintain annual medical calendars, oversee medication management, and complete compliance checks on medication storage and administration.


Key Responsibilities:

  • Coordinate and schedule routine medical appointments, including annual exams, dental visits, eye exams, and specialist visits.
  • Transport individuals to medical appointments, explain procedures calmly and supportively and ensure all follow-up actions are completed.
  • Communicate medical results and instructions to team members, families, and other stakeholders to ensure continuity of care.
  • Record detailed notes from appointments in the appropriate database and maintain up-to-date medical calendars.
  • Manage medication-related tasks, including filling prescriptions, ensuring timely delivery, updating medication administration records (MARs), and conducting regular audits of medication storage areas.
  • Address emergency medical needs by transporting individuals to the ER and following up on treatment plans.
  • Ensure compliance with prior authorizations, pharmacy coordination, and medical procedure preparations.
  • Conduct routine spot checks of PRN medications and MAR documentation to ensure adherence to regulations.

Requirements

Preferred Minimum Qualifications

  • Individual must be of unquestionable character as determined by personal references and background checks. Applicant must supply sufficient recommendations to support the fact that maturity, integrity, and dependability are hallmarks of their personality.
  • Preferably one full year of successful experience working with people with Intellectual and/or other types of Developmental Disabilities.
  • Must have a high school diploma or equivalent.
  • Must have the ability to express oneself in written reports, including effectively electronic reporting of documentation.
  • Past supervisory experience desirable.
  • Must possess good communication skills to convey each person's medical needs to physicians and then communicate needed information back to the Team.
  • Good attention to detail.
  • Ability to use computers to organize and develop schedules.
  • Self-starter and able to work independently.
  • Ability to project a professional image while contacting the community resources.
  • Must have a valid driver's license and acceptable driving record.
  • The ability to communicate effectively, both verbally and in writing, with all levels of co-workers to promote effective teamwork approach to carrying out the responsibilities of the position, programs and the agency's goals.
  • Ability to physically restrain, lift, and/or assist clients.
