We are seeking a full-time Housing Case Manager in the Central Florida Region.
WHAT WILL YOU BE DOING?
- supporting individuals in successfully transitioning into Permanent Supported Housing.
- assisting individuals with independent living, sobriety, psychiatric stability, housing stability and engagement in the community.
- supporting individuals’ ability to remain in housing by assisting them with adhering to lease terms, ensuring compliance with fire and safety standards , and developing positive relationships with neighbors, landlords and the community.
- advocating for, and supporting individuals in self-advocacy related to leasing, fair housing and reasonable accommodations.
- coordination of care for assigned individuals to include assistance with completing referrals to needed services and supporting the individual’s access to available community resources.
- supportive counseling and crisis intervention
- case management
- clinical documentation
Required qualifications: Bachelor’s degree in social work, counseling, psychology or related field. Experience working with individuals with co-occurring mental health and substance use disorders, and/or experiencing homelessness. Must have a valid drivers license and reliable vehicle. Must be able to pass a driving record and criminal background check.
Compensation is $40,040 annually.
