Pathways to Community (PTC) offers an excellent opportunity for a highly motivated Intake Specialist to join our expanding administrative team. In this role, you will be the first point of contact and play a crucial role in maintaining and strengthening PTC’s relationships with both internal and external clients while delivering exceptional customer service. This position involves assessing needs through an intake process, keeping accurate records, and supporting the organization's mission and values to create pathways of opportunity for our community. The role emphasizes building trusting relationships and requires a commitment to diversity and inclusion.

Hourly Range : $27.00 - $29.00 DOQ.
Hours / Days : Core Business Hours : Mon–Fri, 8 : 30 a.m. – 5 : 00 p.m. (evenings and weekends as needed).

Position Responsibilities :

Keep accurate and timely records regarding denials and admissions.

Explain regulations, policies, and procedures as needed to referral sources, Support Team members, PTC staff, etc.

Plan and prepare for all admission meetings by collaborating with the Support Team, Applicant, as well as the PTC Director, General Manager, and the Manager.

Prepare admission documents such as consents, releases, service plans, abuse plans, behavior management plans, etc.

Attend and lead all admission meetings. Obtain signatures on all documents. Finalize all service, abuse, and behavioral management plans and protocols before handing them over to the assigned department.

Education and Experience :

A High School diploma or GED is required.

Working knowledge and experience with / navigating Minnesota Statutes is highly desired.

Experience in the Human Services field and 245A / C / D is preferred.
