We are a contracted partner with AmeriHealth Caritas CHC, Keystone First CHC, PA Health and Wellness, and UPMC HealthChoices. Our mission is to Advocate, Assess, Monitor, and Connect participants to the most appropriate community services that enable and maintain their health, safety, and independence. Role Description
This is a full-time hybrid role as a Service Coordinator at Patient Care Coordination, Inc. The Service Coordinator will be responsible for daily tasks such as coordinating services for participants, maintaining participant-centered services, and collaborating with an interdisciplinary team. The role will be primarily located in Philadelphia, PA, with flexibility for remote work and travel within the 5 county region for participant home visits.
Qualifications
- Service Coordination and Customer Service skills
- Professional Communication both written and verbal
- Time and Organizational Management Skills
- Knowledge of ELTSS, TruCare, Helios, or HCSIS
- Strong interpersonal and communication skills
- Ability to effectively manage and prioritize tasks
- Experience in coordination of community services
- Ability to travel in the 5 county region and long-distances at times
- Knowledge of healthcare systems and regulations
- Experience working with diverse populations
- Proficiency in relevant software and technology
- Bachelor's degree in a related field is required; Master's preferred
