Medication Coordinator

We offer an engaging recreation day program for older adults living with disabilities both physical and cognitive; including, dementia such as Alzheimer’s disease along with health and supportive services to provide compassionate person-centered care.
Essential Duties and Responsibilities Include the following:
Adult Day Care Services
- Perform individual health assessments.
- Design individual care plans in consultation with interdisciplinary team and update every 6 months.
- Monitor participants’ health status on an ongoing basis.
- Counsel participants using nursing intervention techniques.
- Consult with staff, families, and physicians; make recommendations as needed.
- Assist in maintaining participant records.
- Provide medication and diet management.
- Provide emergency care when needed.
- Work as member of interdisciplinary team to maximize benefits of program for participants.
- Participate in weekly staff meetings.
- Assist with participant supervision and personal care.
- Maintain compliance with state regulations and confidentiality.
- Maintain confidentiality.
Skills Needed
· Experience in geriatrics.
· Current CPR and First Aid Certification.
· Ability to prioritize and follow through on details efficiently and effectively.
· Professional demeanor, with the ability to develop and maintain rapport with co-workers.
· California Dept. of Justice and California Dept. of Social Services criminal record clearance required prior to employment.
Experience and Education
· Experience in working with aging population with Dementia and Alzheimer’s disease.
Physical Requirements
· Ability to occasionally lift up to fifty pounds.
· Regularly required to walk, stand, bend, stoop, twist, kneel, reach, and carry.
· Frequently push wheelchairs.
· Ability to assist participants with toileting needs including transfers.
· Able to use computer keyboard, view monitor.
· TB test and fitness for duty exam required.

Founded in 1947, Peninsula Volunteers, Inc (PVI) is a community-based, non-profit organization that develops and administers programs and services for the aging population of the mid-peninsula region, providing over 1,000 hot meals every week to the home bound through Meals on Wheels, and touching over 4500 households through our programs at Little House, The Roslyn G. Morris Activity Center and Rosener House Adult Day Services. With a budget of $5M, PVI has led the nation in developing programs to support the welfare of the older adult members of our community. It is supported by private philanthropy, public grants and fees.

Job Type: Part-time
Pay: $28.00 - $30.00 per hour
