Are you someone that has a strong working knowledge and a passion for mental health and the patients we serve? PCHC is looking to fill a new position, Mental Health Access Coordinator. In this role, you will be responsible for the management of schedules for PCHC’s mental health providers in the Access Center (Program) by providing triage of referrals, communicating processes to patients, scheduling appointments, and working with all sites to ensure timely follow-up and consistency for our patients. Schedule: Full-Time, Monday-Friday, 8am-5pm

Highlights of the position:

  • Embodies PCHC’s mission, vision, and values by treating patients with kindness, respect, and dignity.
  • Manages and coordinates scheduling for all Access Center (Program) appointments including initial intakes, follow ups for patient, and more.
  • Manages in-site referrals via referral tracker and documents all steps taken in that system. Also manages outside referrals and ensures that charts are created and all necessary information is gathered and put in EMR.
  • Provides patients with pre-appointment information and prepare them for process of Access Center (Program) and/or education on what to expect throughout the process.
  • Assists with coordinating referrals and connecting to MH providers at other sites for patients’ whose primary care provider is a part of PCHC and connecting outside referrals with Access Center (Program) therapists for treatment.
  • Assists with training and familiarizing staff to workflows. Participates in hiring, evaluations and providing feedback on processes, workflows, and staff capacity. Engages in ongoing conversation for quality monitoring and development.
  • Is a good steward of health information management. Maintains patient files, records and other information in a safe and confidential format.
  • Serves as a leader to staff and creates a cohesive team environment all while fostering a culture of patient centered care. Collaborates across all teams to improve the patient experience and mental health access processes.

Education and Experience:

  • High school diploma or equivalent required.
  • 2 years’ experience working in a healthcare setting preferred.
  • Experience in working within the mental health field and managing multiple schedules.
  • Stellar customer service skills required.
  • Will have up-to-date training and certification in Healthcare Provider Basic Life Support issued by American Heart Association, American Red Cross or American Health & Safety Institute. Must obtain within 3 months if not current at hire.
