Position Purpose: 

The Community Health Representative is responsible for providing outreach health care services to tribal members and other Native Americans within the local service area. Under the guidance of health professionals, the Community Health Representative provides follow-up and continued care for patients to enhance their health care status and to promote the prevention of disease, maintenance of health and the overall health and well-being of the community.


Essential Duties and Responsibilities:

· Organize work in a timely manner to meet all deadlines and schedules.

· Maintain good working relationship with all Indian and Non-Indian social and health service agencies.

· Refer patient for curative and preventive services.

· Establish home health care by meeting individuals and their families in the home to determine unmet needs.  

· Act as a liaison/advocate for patients with local, state and federal agencies to meet the health care needs of patients.

· Interpret language when the language used by health care providers differs.

· Facilitate communication between patient and providers to enhance accessibility and acceptability of health care facilities.

· Assess an individual’s unmet health and social needs to connect with community and organizational resources, including clear guidance about why and how to access these resources. 

· Document encounters with patients in all assigned systems, contributing and utilizing data to achieve program goals, improve health outcomes, and advance community health indicators. 

· Under guidance of health professional, provide educational training sessions on health issues of tribal concern.

· Provide information and self-help services relating to chronic health problems (i.e., diabetes).

· Participate in food delivery and nutrition education. 

· Schedule and transport patients to and from appointments in adherence to transportation policy. 

· Other duties as assigned.

Requirements

  

Education and Experience:

  • Must possess a High School      Diploma. 
  • Must possess an NREMT Basic Certification      or higher. 
  • CPR Instructor Certification      preferred. 
  • Must have two years of      experience in healthcare related field or equivalent Tribal experience. 
  • Experience in the setting of      Case Management, Nursing (RN, CNA), Family Services, Social Work, Public      Health or Health Coaching is preferred. 
  • Candidate is familiar with      local community resources and health systems. 

Knowledge, Skills and Abilities:

· Must strictly adhere to the confidentiality and HIPAA policies.

· Must have excellent verbal and written communication skills.

· Organizational and record-keeping skills are necessary for accurate maintenance of files, correspondence and follow-up.

· Proficiency with computers and programs like Word, Excel, PowerPoint, EMR and the Internet.


Certifications:

· Valid Driver’s License and Insurance.

· CPR and First Aid Certified within 3 months of hire date.


Conditions of Employment:

The Peoria Tribe of Indians of Oklahoma operates a drug-free workplace.

· Must submit to and pass applicable drug test, TB test, and Physical.

· Must pass background investigations for suitability of character identified in accordance with

the Indian Child Protection and Family Violence Prevention Act, and Child Care and Development Fund, 45 C.F.R. § 98.43(a)(2).

· Adhere to all work rules, policies & procedures, and safety standards.


Work Requirements:

Position involves prolonged sitting, standing, stooping, or bending, lifting, or carrying up to 50 pounds on a daily basis. Work is generally performed in an office environment with varying levels of lighting, air quality, noise, and / or temperature.
