Salary: $60,450 - $63,000/year – based on experience

We are currently seeking a dedicated and passionate Case Manager with experience in Senior Living.

Case Manager Responsibilities include:

• Orient and assist each resident to adjust to life in the community
• Ensure or provide therapeutic interventions to assist residents in coping with their transition and adjustment to facility, including their social, emotional, and psychological needs
• Assist each resident to maintain family and community ties and to develop new ones
• Complete quality management reviews to ascertain if the department is meeting state regulations and PSL policies and follow up as needed
• Work with the care department and others to promote and protect resident rights and the psychological well-being of each resident

Case Manager Education & Experience:

• Bachelors degree with major course work in human services AND one year of full-time work experience in a related field
• Case management experience required
• Memory Care/ Assisted Living experience required
• Compassion for and desire to work with the elderly
