What are the responsibilities and job description for the Registered Nurse (RN), Hospice Admissions position at Phoenix Home Care and Hospice?
Seeking a Registered Nurse to complete Hospice Admissions in the Springfield, MO and surrounding area.
Phoenix Hospice Admissions RN assess and determines hospice appropriateness of patients seeking end of life care. The admission RN should possess strong skills in physical assessment, diagnostic interpretation, communication, and multidisciplinary approach that encompasses the physical, but also the spiritual and psychosocial needs of the patient and their family. Admission nurses work closely with our intake coordinators, hospice consultants and medical directors to ensure a smooth transition from curative care to hospice care.
Availability: Saturday - Sunday, 12-hour shifts & includes on-call responsibilities

Responsibilities:
- Skilled Visits
- Day-to-day patient interaction
- Education on Health Conditions, such as CHF, CODP, Diabetes
- Wound Care
- Medication Management
- Changes dressings, bandages and contraptions as prescribed
- Documentation of patient’s condition and pointing out significant changes, to be reported to the physician.
- Working alongside other nurses, facility staff, physicians, intake coordinators, and hospice consultants
Requirements:
- Previous hospice experience preferred.
- Missouri or Compact Nursing License
- Strong organizational and self-management skills
- Valid driver’s license
