SUMMARY AND SCOPE:

The Tribal Youth Diversion program is an after-school tutoring program that focuses on drug and alcohol abuse prevention for Picayune Rancheria of the Chukchansi Indians youth ages 11-18. The Care Coordinator is responsible for coordinating all system of care services for clients. The Care Coordinator will collaborate with mental health clinical providers and treatment teams in developing care coordination plans. The Care Coordinator will help create crisis plans to address social, economic, academic, familial, cultural, medical, and other issues that impact the clients’ functioning and wellbeing. This position will serve on intervention teams and provides facilitation, as well as advocates on behalf of children, families, and individuals in community settings (e.g. schools, court, county systems, etc.), transport clients, and assists with a variety of client activities based on treatment plan needs. The Care Coordinator is part of a multi-disciplinary team and the majority of services are provided in the field but maybe office-based.


PRIMARY RESPONSIBILITIES:

• Serves as the initial and primary contact with clients of the behavioral health services department and is responsible for coordinating the services for each client assigned;
• Provides in-home, field-based care coordination services and/or office-based services for clients with a range of psychological, emotional, and behavioral problems;
• Provides group educational workshops and presentations to community members as needed per grant requirements;
• Transports clients as needed for activities relating to the care coordination and treatment plan;
• Identifies needed resources for child, family, or individual and provides direct linkages to those programs, assisting the clients with navigating the services if needed;
• Participates as part of a wraparound intervention team as needed for client care;
• Develop and maintain a list of community-based resources and communicates with the Director of Behavioral Health when a memorandum of understanding (MOU) agreement is required or requested to maintain professional referrals relationships between community service providers;
• Completes monthly and annual reporting for grant requirements and PRCI requirements;
• Interface with community agents on behalf of clients and/or the agency (e.g. schools, Department of Child and Family Services, Probation Department, etc.);
• Attends community collaborative meetings and outreach events to share information about PRCI services and reduce stigma around accessing behavioral health services;
• Provides feedback to treatment teams on care coordination issues;
• Provides direct case management services to child, family, individual and identify needs for treatment planning;
• Assists with youth advisory committees;
• Assists with data collection efforts and maintains client chart notes clearly and consistently;
• Conducts data entry into an electronic database that maintains client encounter information;
• Keep Director of Behavioral Health well informed regarding activities, pending issues, and potential problems;
• Participates in agency-wide positive outreach efforts to the community and other community agencies;
• Adheres to agency policies and procedures and advances the goals of PRCI in a manner that embodies the agency’s philosophy;
• Maintains strict confidentiality of client and personnel information adhering to HIPAA, professional codes of conduct, State of CA regulations, Privacy Act, FAIHP policies and procedures, PRCI policies and procedures;
• Other duties as assigned or required to fulfill the purpose of the position.


MINIMUM QUALIFICATIONS:

• Associate of Arts degree from an accredited institution or equivalent.
• Demonstrated experience of a minimum of 1 year in the provision of care coordination or case management services in a clinical or human services setting.
• Strong writing and communication skills.
• Experience engaging community members, specifically in the American Indian/Alaska Native Community.
• Knowledge of Fresno County Behavioral Health System.
• Knowledge of Indian Self-Determination and Education Assistance Act (Public Law 93-638)
• Experience working with the American Indian/Alaska Native community
• Knowledge and understanding of AB3632, probation department, Department of Child and Family Services laws and programs.
• Must be able to pass a background check.
• Must adhere to Tribal drug and alcohol policy.
• Keep regular hours of operations as prescribed by Tribal Administrator and Tribal Council.
• Valid and Clean California Driver’s license.

PHYSICAL DEMANDS:

The physical demands described here are representative of those that must be met by an employee to successfully perform the primary functions of this job. While performing the duties of this job, the employee is required to frequently stand, walk, sit, bend, twist, talk and hear. There are prolonged periods of sitting, keyboarding, reading, as well as driving or riding in transport vehicles. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include reading, distance, computer, and color vision. Talking and hearing are essential to communicate with clients, vendors, and staff.


MENTAL DEMANDS:

There are many deadlines associated with this position. The employee must also multi-task and interact with a wider variety of people on various and, at times, complicated issues.


PRCI TRIBAL PREFERENCE:

For purposes of hiring, promotions, transfers, and training all candidates must possess the ''minimum qualifications" as stated in the job description or job announcement. Minimum qualifications are defined as those entry-level qualifications essential to the performance of the basic responsibilities for each job category, including but not limited to education, training, specific work experience, employment record, and physical skills (where applicable). Preference shall be given with respect to personnel decisions, layoffs, recalls, promotions, transfers, training, and hiring. First, enrolled Tribal Members who meet the minimum qualifications shall not be denied [an interview] if another individual at a lower preference has higher qualifications than are necessary for the position. Second, after preference is provided to enrolled tribal members, Native Americans who are enrolled members of a federally recognized tribe other than Picayune Rancheria of the Chukchansi Indians shall be provided preference over equally qualified non-Indian candidates, however, if the non-Indian candidate is more qualified, a business decision may be made to hire or promote the best candidate. If more than one person at the same preference level meets the minimum qualifications, the decision-makers shall have discretionary authority to make the appropriate business decision in the best interest of the Tribe. Accordingly, when preparing job descriptions or job announcements care should be taken to establish qualifications that fit the desired needs of the position.


NATIVE PREFERENCE:

Under CFR 25, Part 276 and by Title VII of the Civil Rights Act, Section 701(b) and 703(i), preference in filling all vacancies provided to qualified PRCI Tribal Members and/or other American Indian/Alaska Native Candidates.
