Shift and weekend differentials offered
$5,000 Sign on bonus
Schedule: Full time, part time, and per diem positions available!
Morning shifts 6:45a-3:15p
Evening shifts 2:45p-11:15p
Night shifts 10:45p-7:15a
Summary:
Pinnacle Health and Rehab is looking to hireLPN’s for our 72 bed South Portland, ME location. Providing care to our residents within our long term care, hospice, and memory care settings.
Collaborate with a team to develop, implement, and review personalized care plans for residents
Evaluate patients to collect data, observe conditions, and report changes in condition.
Administer medications as prescribed, ensuring accuracy and adherence to protocols
Accurately document and maintain accurate records of medical information, care plans, and incidents to ensure regulatory compliance and consistency of care.
Communicate effectively with residents and residents' families
Advocate for residents' rights and preferences, ensuring their voices are heard and respected in all aspects of care.
Other duties as needed or assigned
Requirements:
High School Diploma
Current Maine LPN
Experience working within a nursing home, assisted living, or hospital (preferred)
