POSITION SUMMARY
The Day Program Client Care Coordinator supports adults with Intellectual Disabilities that are served at Pioneer Center Day Programs. The Client Care Coordinator serves as the primary advocate for each client in their caseload, ensuring not only compliance, but quality of life and general happiness within the Day Program. The Client Care Coordinator collects the raw data necessary to evaluate program effectiveness, efficiency, and stakeholder satisfaction. The Client Care Coordinator provides necessary services to ensure a client’s progress towards goals set for them through the Person-Centered Planning Process.

Essential Functions Statement(s)
- Carries a caseload within DHS community day guidelines.
- Establish contact with guardians and/or family members. Facilitates communication between guardians and/or families and relevant agency staff.
- Responsible for completion of monthly service reviews, Implementation Strategies, ICAPs, Monthly Reviews, CIRs, progress notes/billing, and consents. Enter all required information/assessments via the Agency’s EHR system, where and when appropriate.
- Possess knowledge of entitlements and be a resource to clients, families, and other agencies.
- Must prepare all relevant paperwork and present at Human Rights Committee (HRC) as needed.
- Ensures compliance with Auditors such as BQM, BALC, CARF, HCBS (DHS), etc.
- Responsible for the generation and collection of program data. Duties include writing client goals, training staff on goals, monitoring, and collection of data.
- Informs the direct supervisor of all conditions that may prove detrimental to the clients well being.
- Conducts all phases of client service in compliance with the laws on confidentiality and HIPAA.
- Leads a program of which the topic is based upon HCBS Settings Rules requirements for the Day Program.
- Must possess excellent verbal and written communication skills.
- Regular, reliable, predictable attendance.
- Maintain continuing education as required per DHS once QIDP certification is obtained.
- Performs all other duties as assigned by direct supervisor.

SKILLS & ABILITIES
Education: Bachelor's Degree (four-year college or technical school) Required, Field of Study: Human Services can include but not limited to sociology, special education, psychology, and criminal justice.
Experience: 1 plus years of experience working directly with individuals with a developmental disability.
Computer Skills: Microsoft Office, Word, Excel, PowerPoint, Outlook, Microsoft Teams, and SmartCare
Certifications &
Licenses: QIDP Certification or eligible to become a QIDP following Department of Human Services standards.
Other Requirements: Must have and maintain a valid driver's license.
Must obtain twelve (12) CEU's annually in order to maintain QIDP certification.
Ability to use Phone, Fax, Copier, and file required.
Must be able to meet agency driver safety requirement of carrying personal vehicle insurance with the following limits:
