FLSA Status: Exempt      Reports To: Residential Services Coordinator 
Grade/Level: 5      Amount of Travel Required: 50%  
Job Type: Regular      Positions Supervised:  DSPs  
Work Schedule:       
Monday – Friday 8am- 4pm

POSITION SUMMARY

The Program Supervisor is responsible for the compliance, administration, and day-to-day operation of the program(s) in which they oversee in the Community Day Services Program

Essential Functions Statement(s)


Management


•  Work with Operations Coordinator to fill/solve any scheduling gaps by utilizing the approved staff e.g.,      subs, part-time, floaters.
•    Performs personal care tasks, including assistance with basic personal hygiene and grooming, toileting, feeding, and ambulation, medical monitoring, and health care related tasks. 
•    With agency vehicle, transports individuals to/from community activities as needed.
•    Fosters positive relationships with individuals served. 
•    Ensures client safety and maintaining a safe environment. 
•  Administer medications and first aid as needed.
•  Facilitates program committees.
•  Ensures programmatic standards are met, including community inclusion and developmental training.
•  Review the weekly schedule to ensure all shifts are filled.
•  Complete monthly training with all staff, including checklist sign-off sheets for all staff. 
•  Communicate on an on-going basis with QIDP’s.
•  Working with the QIDP, ensure a deep understanding of clients’ goals, which are required to be tracked   
   and documented within the program   
•  Communicate daily all client issues, successes and other relevant information to the QIDP or program  
   Coordinators 
•  Working with the QIDP, provide input and direct implementation of behavior plans within the program, as   
   needed


Administrative 

•  Audit data sheets monthly to ensure accurate completion by program staff.
•             Maintains client confidentiality and respects client boundaries.
•             Follows HR policies and completes HR paperwork. Contacts HR with questions concerning either.
•             Performs all other duties as assigned by Supervisor.
•             Regular, reliable, predictable attendance.
•            On a monthly basis, completes activity calendars and ensures implementation by staff
•            Manages attendance and/or enters into Smartcare.
•            Provides quarterly inventory of program supplies

SKILLS & ABILITIES

Education:    High School Graduate or General Education Degree (GED): Required 
Bachelor's Degree (four-year college or technical school): Preferred 


Experience:    2 plus years of experience in a progressive leadership role 
1 plus years of experience in the DD field 


Computer Skills:    Evolv, Microsoft Office, Word, Excel, PowerPoint, Outlook  
 

Certifications &
Licenses:     DSP Certified
 

Other Requirements:    Must have and maintain a valid driver's license. 

Must be able to meet agency driver safety requirement of carrying personal vehicle insurance with the following limits:
$50,000 bodily injury, each person
$100,000 bodily injury, each accident
$50,000 property damage

Ability to use Phone, Fax, Copier, and file required.

General office environment. Weather and road conditions can be a factor while driving for the position. Frequent interaction with any stakeholder within the agency (i.e. supervisors, co-workers, clients, general public, outside agencies, etc.).
