Physician - Vasectomy Provider

Planned Parenthood Arizona (PPAZ) is Arizona’s largest reproductive health care provider, serving over 34,000 patients every year. GENERAL PRIORITIES

• Perform vasectomy procedures.

• Supervise post-procedure care until all patients are stable and/or discharged as defined by protocol.

• Order post-procedure testing, including post-vasectomy semen analysis. Participate in review and management of post-vasectomy semen analysis results. 

• Comply with all informed consent, mandated waiting periods and parental consent notification laws. Document compliance with all laws. Ensure consents properly signed and in order prior to performance of procedure.

• Obtain a pre-procedure history, physical examination, and appropriate laboratory tests as indicated.

Qualifications

EDUCATION AND/OR EXPERIENCE

• Doctor of Medicine - Certified MD or DO from an accredited medical or osteopathic school.
• Licensed to practice medicine in Arizona.
• BLS certification.
• Privileged to perform vasectomies based on prior experience and/or training.
