Occupational Therapy Assistant
Polaris Health Care and Rehabilitation Center is looking for an experienced Occupational Therapy Assistant to join our rehabilitation team and provide therapy services to patients in our skilled nursing facility. We are looking for someone with experience working in a skilled nursing facility . The right candidate is patient, compassionate and professional who encourages and inspires their patients to work toward greater independence and recovery.
Role of Occupational Therapy Assistant
- Implement an individualized treatment plan based on patient assessment provided by the Occupational Therapist to restore function and independence
- Monitor patients' performance in therapy activities and adjust treatment program to obtain improved progress
- Collaborate with the interdisciplinary team members to provide input regarding patient needs and establish a rehabilitation plan.
- Maintain positive interactions with patients, families and caregivers while providing instruction and education in home programs, basic living skills, and use of adaptive equipment.
Qualifications for Occupational Therapy Assistant:
- Experience providing occupational therapy in a skilled nursing facility
- Graduated from an accredited Occupational Therapy Assistant program
- Current Occupational Therapy Assistant license for state of Delaware
- Current CPR certificate
- Requires good organizational and time management skills
- Requires good verbal and written communication skills
Salary : $45,800 - $58,000
