SUMMARY:

The Caregiver Coordinator will provide temporary assistance to unpaid individuals caring for an elder; or grandparents and/or older individuals who act as the primary caregiver for children under the age of eighteen.

ESSENTIAL DUTIES:

  • Locate and identify individuals and families that fall within the project guidelines.
  • Provide information to caregivers about available services.
  • Provide caregivers assistance in gaining access to the available services.
  • Provide caregiver training to assist them in making decisions and solving problems relating to their care giving roles.
  • Provide respite care to enable caregivers to be temporarily relieved from their care giving responsibilities.
  • Coordinate services with PBP Health Promotion Team to ensure adequate services are provided.
  • Adhere to policies and procedures set forth by project guidelines.
  • Provide written monthly reports to the Elder Center Manager, quarterly and annual reports to Administration, and annual report to Title VI Administration on Aging.
  • Provide documentation of services provided to the Elder Center Manager.
  • Assist with Elder Center activities, meal prep, and home delivery when needed.
  • Annual competency evaluation.
  • Works cooperatively within the office, Prairie Band Potawatomi Nation, and other agencies; as needed
  • All other duties as assigned.

MINIMUM REQUIREMENTS:

  • High School Diploma or GED
  • Less than one year of experience in working with and or providing assistance to elders
  • Must have a valid Kansas driver's license and be insurable with the Nation's insurance
  • Must obtain CPR certification within six (6) months of hire
  • Must be able to become an appointed Notary Public through the State of Kansas within six (6) months of hire
  • Ability to make decisions on the best care for patients; recognizing who needs services and abilities to determine what services they are eligible for
  • Ability to operate general office equipment
  • Must be available to work the assigned schedule of M-F, 8:00A-4:30P and available as needed to work a flexible schedule to meet the demands of the position, which may include nights, weekends and holidays
  • Must be willing to travel on the job, 50% of the time
  • Must be reliable, punctual and dependable in order to meet the needs of the department and Prairie Band Potawatomi Nation
  • Must possess professional work ethics, manner, and appearance
  • High level of interpersonal skills to handle sensitive and confidential situation and documentation
  • Ability to maintain high level of confidentiality
  • Must possess ability to communicate professionally; both orally and in writing a service-orientated approach, sometimes in stressful circumstances
  • Must possess strong organizational skills with the ability to prioritize work and meet deadlines with very little supervision
  • A record of satisfactory attendance and performance in all prior and current employment

PREFERRED QUALIFICATIONS:

  • High School Diploma or GED with Home Health and or CNA Training
  • Notary Public through the State of Kansas
  • Valid CPR certification
  • One to two years of experience in working with and or providing assistance to elders

PHYSICAL, MENTAL, & ENVIRONMENTAL DEMANDS:

  • Ability to develop positive working relationships with others, including members of the public
  • Ability to maintain high level of privacy and confidentiality of individuals and families
  • Ability to operate general office equipment, including but not limited to: 10-key calculator, telephone, computer and operating systems, computer software programs (Microsoft Office programs), fax/copier machine
  • Able to read, write and speak English
  • Ability to work efficiently in a highly team-team oriented environment
  • Ability to work in a high-stress, deadline oriented environment
  • Attend out-of-state/out-of-town training periodically to enhance job performance and knowledge.
  • While performing the duties of this job, the employee is regularly required to have full range of mobility in upper and lower body; be able to reach overhead; be able to work in various positions, including stooping, standing, bending over, sitting, kneeling and squatting for extended periods of time; be able to lift, pull and push materials and equipment to complete assigned job tasks; and be able to frequently lift 21-50 pounds of weight throughout the assigned workday
  • Safety/Health Hazards: Exposure to illnesses and diseases

  • Background Investigation with adjudication
  • Pre-employment Drug Screening
  • Tuberculosis (TB) Screening
