Job description
ALL APPLICANTS MUST HAVE LPN professional license and 1 year Home Care experience. Ability to travel to cases as assigned. Job Summary: Provide quality nursing care to patients in various settings in the community. In collaboration with the RN case manager to provide skilled nursing visits. Utilizing thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and standards of practice of Home Care.
Qualifications
EDUCATION, EXPERIENCE, TRAINING
1. 1 year Traveler Experience
2. 1 year Home Care experience.
3. 1 year Case Management Experience
4. Current licensure in the state of Massachusetts
5. Reliable, independent means of transportation to make home visits.
6. Must be willing to work weekends/holidays/on call in accordance with department need/rotation Responsibilities:
Location: 8635 W. 3rd Street, Los Angeles, CA 90048
Float Location: 1192 Hilhurst Ave, Los Feliz, CA 90027
Shift: 8 hours between 0600-1800
***Experience with the following OBGYN procedures required: PAP, Coplo, Leep, D&C, EMB, IUD insertions, and removals, Nexplanon, and NST***
Required:
* CA License
* BLS
* IV/Blood Draw Certified
* 1 Year of OBGYN experience
* 2 Years of overall LVN experience
