Premier Choice Senior Management is looking for a dynamic and compassionate Registered Nurse to join their team to be a clinical leader supportive various senior care clients ranging from Crosslake MN to Sandstone MN.
Duties: The following duties are normal for this position, other duties may be required and assigned, this list is not all inclusive.
1. Performs supervisory duties for Assisted Living, Memory Care and Specialty Care Attendants, including instruction, assigning and reviewing work, planning, maintaining standards; acting on employee problems and discipline; promotes staff development and motivation; provides instruction and guidance; and performs employee evaluations.
2. Oversees, monitors, and checks on health needs and care of residents including
medication administration, dressing changes, specialty diets.
3. Schedules staff work. Shared with administrator.
5. Participates in staff and resident evaluation meetings.
6. Interacts with residents, families, and health care workers.
7. Responds to facility emergency calls on an on-call rotation.
8. Notes safety hazards in resident’s apartments and monitors and provides safety instruction for residents.
9. Performs health updates and schedules assessment, interviews, and charting, maintains medication records, outpatient logs and takes doctors’ orders.
10. Answers telephone and assists visitors at the facility.
11. Provides in service training of all care providers.
12. Schedules transportation arrangements for residents to doctors, etc.
13. Attends in-service training workshops and meetings, when possible.
14. Conducts work tasks safely and in compliance with the facility safety program reports to administrator as well as cooperate RN.
15. Provides effective and courteous service to all residents, guests and coworkers.
16. Promotes and protects the rights of each resident.
17. Performs other related essential duties as required.
18. Works with administrator and monitor of the AWAIR program.
19. Works with administrator HIPAA program.
20. Works with administrator on all admissions and makes sure the Service Plan is followed
through with staff.
21. Covers shift work if an employee does not show up to work, shared with administrator.
22. Monitors resident’s charts and clinical progress notes.
23. Follows through admission process with each resident and assists him or her during
the transition periods.
24. Informs the physicians, administrator, and other personnel (county case managers) of changes in
resident’s needs. Tracks all referrals and follows up.
25. Is willing to carry cell phone for on call emergencies.
26. Coordinates staff education to include 8 hours or more hours of in services annually.
27. Assures facility is in compliance with state, Federal and local standards and
regulations. Is available for licensure surveys.
28. Staff scheduling, shared with admin.
MINIMUM TRAINING AND EXPERIENCE
Registered Nurse License with three to five years of prior nursing experience, some geriatric experience preferred, or any other equivalent combination of training and experience which provides the required skills, knowledge and abilities.
KNOWLEDGE AND ABILITIES REQUIRED TO PERFORM ESSENTIAL JOB FUNCTIONS
Knowledge of:
1. Nursing, nursing theory and practical applications as they relate to the medical problems experienced by residents for whom care is provided.
2. Policies and procedures concerning resident care and administering them effectively.
3. All procedures required to perform, and skill in using, a variety of medical equipment
Efficiently and carefully.
4. Record keeping duties regarding residents and employees that must be performed.
5. Medications and methods of care in administering same to residents.
6. Policies, procedures, and techniques of effective supervision.
Physical Requirements:
1. Ability to operate a variety of automated office machines including word processor, typewriter, calculator, copier, personal computer, etc.
2. Ability to operate and use a variety of health care equipment and tools including
Stethoscope, blood pressure equipment, otoscope, Acucheck, syringes, thermometers,
Oxygène tanks, etc.
3. Ability to exert a small amount of force occasionally and/or a moderate amount of
Force frequently or constantly to lift, carry, push, pull or otherwise move objects.
4. Must be able to lift and/or carry weights of residents in positioning and transporting
them.
5. Work involves walking and standing for prolonged periods of time.
6. Ability to differentiate between colors, sound, texture, and odor.
7. Must be able to stand, sit, bend, stoop squat, twist and be able to lift a minimum of 50 pounds consistently to meet job requirements. Must not have any weight restrictions.
Supervisory Responsibilities:
1. Ability to assign, review, plan and coordinate the work of other employees.
2. Ability to provide instruction to other employees.
3. Ability to recommend the discipline or discharge of other employees.
4. Ability to recommend the transfer, promotion or salary increase of other employees.

5. Ability to use independent judgment and discretion in supervising department activities, handling difficult situations, handling residents’ complaints, maintaining standards, and providing assistance and information to residents and their families.
Language Ability and Interpersonal Communication:
Ability to comprehend and correctly use a variety of informational documents including resident records, lab reports, Resident Assessment response forms, X-ray reports, timecards, job applications and other reports and records.
Ability to prepare transportation sheets, monthly schedules, doctor’s report sheets, timecards, order forms, correspondence, and other job related documents using prescribed format and conforming to all rules of punctuation, grammar, diction and style.
Ability to comprehend a variety of reference books and manuals including drug books, medical dictionary and various diagnostic and gerontological reference materials.
Ability to record and deliver information, to explain procedures, to maintain confidentiality of restricted information, and to follow instructions.
Ability to use and interpret medical terminology and language.
Ability to communicate effectively with residents, doctors, families, supervisor, maintenance and housekeeping personnel, social workers, physical therapists, coworkers, staff and general public, verbally and in writing.
Environment Adaptability:
Ability to work effectively in residents’ apartments and in the assisted living facility.
Ability to work in a health care environment with possible exposure to disease, odors and toxic substances.
Ability to work effectively in resident’s apartment and environment that may have animal hair and or animal dander
Qualifications:
- Valid Registered Nurse (RN) license in the state of practice
- Bachelor's degree in Nursing preferred, but Associate's degree or diploma in Nursing accepted
- Minimum of 5 years of nursing experience in a similar setting (e.g., hospital, clinic, long-term care facility)
- Strong knowledge of medical procedures, treatments, and medications
- Excellent clinical assessment skills and ability to make sound decisions in emergency situations
- Proficient in using electronic health record (EHR) systems, such as Athenahealth or similar platforms
- Ability to effectively communicate with patients, families, and healthcare team members
- Compassionate nature and strong commitment to providing high-quality patient care
