Premier Home Health Care Services, Inc. is a leading established Home Health Care Agency. We currently have an opportunity for a Care Coordinator in South Chicago, IL.
We are looking for a dynamic individual to conduct determination of eligibility / need assessments, facility placement pre-screening assessments, and authorize Community Coordination Program (CCP) Services for all eligible Illinois Department on Aging’s CCP program participants.
Key Functions include :
- Administration of the appropriate intake form, including a comprehensive needs assessment.
- Development of a case plan.
- Authorization of services.
- Maintaining case records, including documentation of follow-up, reassessment, and termination.
Qualifications :
- Must possess a BA / BS degree in social science, social work or related field.
- Two years of program experience, which is defined as assessment, provision, and / or authorization of formal services for the elderly.
