Youth & Family Support Specialist

Essential Duties of this Role
To assure the safety of children and youth , the YFS will:
- Be available at scheduled hours to assist with maintaining visual and auditory contact with youth during home visits, in-service, and community activities;
- Assist in identifying potential safety hazards to the youth and taking necessary steps to eliminate or minimize injury.
To assist in implementation of program activities the YFS, as needed, will:
- Transport youth to family visits, external appointments, and other activities when needed and as appropriate to the service being delivered;
- Monitor and/or supervise biological family contact as per court order;
- In TFC, assist youth with Independent Living Skills;
- Support and guide youth while participating in peer activities;
- Participate in program activities with the youth as needed;
- Communicate any/all information pertaining to a youth, including prescheduled appointments, to program staff;
- Deliver services as outlined in service plan/ISP;
- Support staff in job related functions and units of service.
To assist in arranging for goal driven activities "in and out" of the home, the YFS will:
- Be available to provide transportation as needed and as appropriate to the service being delivered;
- Be available to relieve staff or TFC parents , as the schedule requires
- Work alongside families, staff, and/or TFC parents, to support completion of goals and activities
To assure appropriate documentation of program activities, the YFS will:
- Document time spent with youth as required by program and regulation, which may include what activity was performed, time of activity, and participants;
- Complete written work that is legible, accurate and use professional language meet timelines for submitting paperwork established by the program and organization;
- Gather program data, as needed, and assist in preparation of data and/or summarizing information;
- Complete other required paperwork and documentation
To promote his/her professional growth and development, the YFS will:
- Attend appropriate in-service, staff training , SWAN trainings, or other required job related internal and external training; Participate in supervision .
Experience
Required- 1 year(s): Experience working with children and/or adults
Education
Required- High School or better
