Prestige is seeking a Community Care Liaison who is passionate, caring and supportive to act as a liasison and ambassador for the services we provide.
If you have previous experience working with developmentally disabled adults in a Director OF DSP SERVICES or Support Coordination position we encourage you to apply. Responsibilities :
- Community outreach
- Establish and maintain a positive rapport with referring agency personnel in order to build productive, consistently high referral numbers.
- Identifying Individuals that can benefit from Prestige.
- Collaborate with DDD and Support Coordination
- Document to ensure State and Federal Compliance
- Assist with implementing new DDD poolicies
