POSITION DESCRIPTION:
Provider Relations Representatives - Practice Transformation Consultants (PRC) are highly visible, hands-on personnel responsible for assisting physician offices with achieving healthcare improvement initiatives, provider metrics and workflow optimization with the goal of better patient health, value and rewards. These individuals are responsible for managing all aspects of practice engagement to ensure quality and value timelines are met. PRCs are responsible for business development activities including practice satisfaction and retention and supporting the Network Development department’s expansion of services to the practices. PRCs are a partner, coach and mentor to practices and accountable for engaging and motivating practices to achieve results. With each practice being unique, PR team members use their social and analytical problem-solving skills to develop relationships of trust, understanding and engagement to action.
RESPONSIBILITIES:
- Develop and facilitate Primary Partners and Practice approval and signoff and then execute and report on progress to meeting an annual practice Key Focus Action Plan (KFAP). A KFAP is a plan on how a Practice will achieve the requirements of participation in contracts.
- Serve as primary contact with the practices and assist them in transforming their practices to improve quality, operations and generate incentives through quality and shared savings programs
- Provide onsite support to practices and their office managers to improve their understanding of all accountable care reports, programs and initiatives needed to meet their KFAP goals
- Educate providers about the comprehensive policies and procedures around population health management.
- Assist the practices in streamlining workflows to support accountable care initiatives.
- Review reports and communicate regularly with providers and staff to ensure higher levels of understanding, engagement, participation and result achievement.
- Build trust, relationships with administrative, clinical and provider personnel which supports practice growth and retention
QUALIFICATIONS/KNOWLEDGE/SKILLS REQUIRED:
- Self-starter, self-motivated, individual with ability to build portfolio of high performing practices
- Socially adept in engaging and motivating others
- Organized with the ability to prioritize workload, multi-task and meet deadlines
- Aptitude to understand complex situations and bring unique solutions which facilitate practices achieving results
- Comfortable with using and leveraging technology
- Proven ability to work with diverse skillsets and individuals such as physician, clinical and administrative personnel
- Collaborator, sharer of best practices and supporter of fellow team members
- Driven to get results, passionate about making a difference in the health and accessibility of care to the communities and patients served
- Ability to build relationships and leverage other department personnel and their expertise
- Comfortable asking questions, pointing out challenges and bringing up solutions in meetings or one on one
- Excellent verbal and written communication skills
- Reliable transportation and valid driver’s license for office and site visits

EDUCATION:

EXPERIENCE:
