Description
Family Care of Enfield and Family Care of South Windsor are looking for an APRN who is a team player and passionate about patient care to join our teams! We are two privately owned family practice offices located in Enfield and South Windsor with a potential third location in Avon, CT. Four days per week.
Job roles include but are not limited to:
o Functions independently to perform age-appropriate history and physical for complex acute, critical, and chronically ill patients
o Performs annual wellness visits for Medicare patients
o Performs preoperative physicals
o Orders and interprets diagnostic and therapeutic tests relative to patient’s specific needs
o Prescribes appropriate medications
o Administer injections.
o Perform and/or assist minor surgical procedures as qualified.
o Obtain wound and other cultures as indicated.
o Collaborates with multidisciplinary team members by making appropriate referrals.
o Facilitates staff, patient and family decision making by providing educational tools
Requirements
o Core Competencies/Skill Sets
o Strong communication skills
o Able to use sound judgment
o Bilingual (English and Spanish) skill preferred
Professional Experience/Educational Requirement:
o Registered Nurse in the State of CT
o Healthcare experience
o Certification/Licensure
o Current Nurse Practitioner certification
o Current provider card in Basic Life Support (BLS); CPR certified
Salary : $90,000 - $120,000
