SUMMARY: Provides support and follow-up to clients and outreach to the community.
RESPONSIBILITIES:
- Responsible for chart review including service plan outcomes and time frames, timeliness of service provision, number of client contacts, etc.
- Monitoring and updating chart documentation.
- Direct care services (e.g. providing referrals for medical appointments, hospital visitation and client escort as needed)
- Scheduling of client's appointments and participation in case conferences.
- Escort clients to medical appointments & conducting home visits to follow up with clients
- Follow-up with regard to scheduled appointments and follow-up on outcomes of goal-related activities.
- Provide client assistance services, health education group facilitation, case management support to Clinical team, client re-engagement and outreach activities, and case finding activities.
Patient Navigator provides direct support services to client, including by not limited to accompaniments to appointments, treatment adherence support, facilitation of client education groups and outreach activities, client assistance around management of medical appointments and other social services appointments, case management support to program team and chart management activities. The PN would be providing direct client services.
Qualifications
At least 3 years prior experience in social services, working with PLWH and Substance Use required. Minimum HS/GED. Bilingual and lived experience preferred but not required.
KNOWLEDGE:
Position requires High School Diploma or equivalent. At least 3 years prior experience in social services, working with PLWH and Substance Use required. Experience working with chronic illness/HIV positive/Substance Abuse/Mental Health population preferred. Bilingual Spanish/English a plus. Clean and valid driver’s license needed. Must have exceptional communication skills both orally and in writing, with individuals and groups regarding complex and sensitive issues. Physical ability to perform the essential job functions are needed.
