About The Position
As a Community Inclusion Specialist, you will play a vital role in supporting individuals as they transition from adult care homes into permanent supportive housing. By fostering connections to services, resources, and their chosen communities, you’ll help individuals decrease isolation, pursue employment and education, and enhance their quality of life. Grounded in PRN’s mission, you will use compassion, collaboration, and trauma-informed practices to drive lasting change.

About The Position
As a Community Inclusion Specialist, you will play a vital role in supporting individuals as they transition from adult care homes into permanent supportive housing. By fostering connections to services, resources, and their chosen communities, you’ll help individuals decrease isolation, pursue employment and education, and enhance their quality of life. Grounded in PRN’s mission, you will use compassion, collaboration, and trauma-informed practices to drive lasting change.
What You’ll Do
Program Support
- Follow through on referrals to assist individuals receiving Community Inclusion Peer Support or Transition Services.
- Complete assessments and tools to identify needs and opportunities, adhering to best practices like “Mattering” from Temple University.
- Provide reporting and billing information to ensure accurate documentation and reimbursement.
- Work with the Program Manager and Alliance staff to coordinate efforts for successful community transitions.
- Adhere to person-centered, strength-based principles to help individuals achieve their goals.
- Ensure safe, affordable housing options and connect individuals with necessary benefits and services.
- Provide training to enhance community living skills.
- Educate individuals about housing, services, and opportunities for meaningful community inclusion.
- Engage with partners and external stakeholders to encourage referrals and service engagement.
- Support individuals in building relationships with peers, accessing supports, and exploring community resources.
- Collaborate with organizations, nonprofits, and housing specialists to connect individuals with resources like housing and home furnishings.
- Participate in weekly program meetings to align on goals and share updates.
- Current North Carolina Certified Peer Support Specialist.
- Personal experience in recovery and a commitment to wellness.
- Strong belief in the recovery potential of all people and the philosophy of peer support.
- Ability to manage time and tasks independently while receiving guidance and mentoring.
- Excellent communication skills, including active listening, conflict resolution, and non-judgmental support.
- Proficiency in computer applications (word processing, spreadsheets, databases, presentations).
- Valid NC Driver’s License.
- Demonstrates honesty, integrity, and the ability to work collaboratively with stakeholders.
