Description
Reporting to the Director of Pharmacy, this position is responsible for providing and coordinating pharmaceutical care services and collaborative drug therapy management for patients. The Clinical Pharmacist will serve as a resource to the patients, physicians, pharmacists, nurses and other members of the health care team. The clinical pharmacist; 1) assures safe and effective medication use and; 2) demonstrates proficiency in delivering care to assigned age specific patient population (e.g., infants, adolescents, adults, geriatrics, and pediatrics); and 3) participates in related continuing education.

Required Qualifications:
PharmD from an Accredited College of Pharmacy.
Bachelor's Degree in Pharmacy-Acceptable for employees hired prior to the revision date of this Job Description (10/01/12).
Upon hire: California Pharmacist Registered.
California Fire and Life Safety Card - National Organization within 30 days of hire.
Preferred Qualifications:
- 2 years Acute care experience or a Pharmacy Residency equivalent to 3 years of pharmacy practice experience.
