Psychotherapeutic Services is seeking a dedicated Psychiatric Rehabilitation Specialist to join our Team in the Burlington
The mission of the Psychiatric Rehabilitation Specialist is to assist persons served by providing services and supports necessary to assist the recipient in achieving and maintaining rehabilitative, sobriety, and recovery goals. This is intended to meet the mental health or substance abuse needs of adults who have significant functional impairments that seriously interfere with or impede their roles or functioning in family, school, or community.
Tasks include:
- Completes assessments, interviews, and contacts persons served as required by program or contract.
- Links persons served with community resources.
- Links persons served with housing resources to assist with living conditions.
- Works with the persons served to plan, advocate, and monitor person’s needs.
- Assists persons served in obtaining entitlements.
- Provides support and assistance to persons served to assure continuity of care.
- Performs case management services at various locations – within the community, in client homes, etc.
- Assists persons served to become independent and to encourage efforts towards recovery & rehabilitation.
- Evaluates and monitors persons served behaviors, applying evidenced based interventions in accordance with training and credentials.
- Documents all interactions, observations, assessments, plans, etc. Completes paperwork and reports as assigned.
- Develops service and treatment plans as required and establishes applicable goals, outcome measures, and time frames for completion.
- Coordinates/monitors medical & psychiatric needs.
- Performs Record Notation/documentation
EDUCATION & EXPERIENCE:
Bachelor’s prepared psychiatric rehabilitation specialist. Three (3) years’ experience providing individual care in a related field, one (1) year supervisory experience and experience serving persons with severe and persistent mental illness/ substance abuse is preferred.
Salary : $50,000 - $70,000
