Job Overview
The Referral Coordinator is responsible for making specialist appointments, tracking and following up with heath center patients. The Referral Coordinator will report to the Clinical Director or designee.
Essential duties and responsibilities
- Schedule referral appointments for all center patients.
- Maintain competency in all aspects of the Children’s Health Fund Referral Management System.
- Complete required referral forms.
- Provide directions to referral appointments, as needed.
- Send reminder letters for appointments.
- Contact patients to remind them of appointment the business day before the appointment.
- Call place of referral and determine if appointment was kept on day of appointment. If appointment was not kept, re-initiate process.
- Remind and monitor referral source regarding the need to have paperwork describing the results returned to.
- Enter completed referral data into the EMR, including the appointment time, data and location.
- Scan specialist reports into EMR.
- Track all referrals in the PM system
- Will function as dual coverage for the front desk as well
- Perform other duties as assigned
- Place reminder appointment calls
Skills:
- Knowledgeable about specialist referrals.
- Data entry experience, particularly with practice management system
- Knowledge of medical terminology
- Must have pleasant manner
Experience:
- One year experience working either as a Medical Receptionist or a Referrals Coordinator in a primary care setting.
Education:
- High School Graduate
Salary:
- Based on experience and Salary grade
Salary : $32,500 - $41,200
