What are the responsibilities and job description for the Case Manager position at Qualified Healthcare?
Responsibilities:
- Provide case management services to individuals and families in need
- Conduct assessments to determine client needs and develop individualized service plans
- Assist clients in accessing and navigating various social service programs, such as workforce development, Section 8 housing, Medicare, and childcare assistance
- Advocate for clients and coordinate with other service providers to ensure comprehensive care
- Utilize motivational interviewing techniques to empower clients and promote self-sufficiency
- Monitor client progress and make necessary adjustments to service plans
- Provide crisis intervention and support during emergencies or challenging situations
- Maintain accurate and up-to-date case records
Experience:
- Bachelor's degree in social work or a related field preferred
- Previous experience working in a case management role, particularly in the areas of child welfare or workforce development, is highly desirable
- Knowledge of community resources and social service programs
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Excellent organizational skills and attention to detail
- Experience with supervising or mentoring others is a plus
