What are the responsibilities and job description for the Bilingual Care Coordinator position at Qualis Home Health Services, LLC?
Care Coordinator – Key responsibilities include, but are not limited to:
Referrals
- Receive all new referrals to initiate the intake process (intake assessment form)
- Obtain service authorization requests from non-Medicare pay sources and ensure that such authorizations for service are in place for the first and subsequent episodes of treatment periods
- Confirm insurance verification of all new and current applicable cases
- Oversee the third-party (MCO’s) credentialing process as well as ensure credentialing maintenance is carried out
- Coordinates with the Billing Coordinator, to ensure required documentation is obtained from physicians, support coordinators and case managers, etc.
- Oversee and maintain referral portal and conduct routine warm calling to convert a referral into a new client
- Oversee all aspects of obtaining service authorizations as well as annual service authorization renewals
Supervision
- Serve as the immediate supervisor of assigned team of Caregivers
- Coordinate and oversee in-person and virtual "Meet & Greets" between the assigned Caregiver(s) and the Client/Family
- Communicate, on a routine basis, with contract agencies, to maintain smooth relations and to obtain needed information
- Serve as the first point of contact for all Client/Family Needs
- Assist with scheduling to ensure the most appropriate assignment of caregivers for each shift and make recommendations in the event a caregiver reassignment is necessary
- Collaborate with clinical team to schedule new client and renewal assessments
- Ensure the client plan of care is effectively carried out
Administrative
- Data entry and oversight of various client platforms
- Create and maintain weekly and quarterly reports
- Partake in monthly caregiver meetings
- Ensure that all new client forms such as the consent for treatment and service agreement are completed (either electronically or manually), and the completed documents are saved electronically or ensure the originals manually completed returned back to the office
- Assist with on-call after hours calls (clients and/or Caregivers 1-2 weeks a month) (including a minimum of two assigned weekends per month)
- Answering multiple phone lines, generating emails, and faxing/scanning of documents
Job Type: Full-time
Pay: $22.00 - $25.00 per hour
Expected hours: No more than 40 per week
